Are you passionate about supporting older people to live well and independently? We are looking for a dedicated and experienced Care Partner to join our Ageing Well team.

This role is ideal for someone who communicates in Auslan, thrives in a fast-paced environment, and brings strong experience in the Support at Home program.

This position supports clients across NSW, VIC, SA and WA, so flexibility and the ability to manage a broad regional caseload is essential. You will be required to travel by car to visit clients within the region you are located in and for other states it will be via the virtual platform.

About the role:

As a Care Partner, you will support older Australian clients to achieve their goals and maintain their wellbeing under the Support at Home & CHSP programs.

Main responsibilities of the role include: 

  • Making sure each client has an approved and up-to-date care plan
  • Developing action plans that address assessed care needs
  • Matching supports to the client’s approved budget
  • Implementing and reviewing care plans in a timely, person-centred way
  • Completing mandatory monthly care management activities
  • Maintaining accurate case notes and documentation
  • Making referrals to external service providers
  • Monitoring services and adjusting supports where needed
  • Working collaboratively with allied health professionals, carers, families and community supports
  • Supporting compliance with aged care regulations and Support at Home program standards
  • Assisting to maintain and grow the participant base

You will also support the Manager, Ageing Well to ensure services are high quality, compliant and responsive to community needs. The role may evolve over time to meet business and program requirements.

Please note: this is a fast-paced role requiring strong organisational skills and the ability to manage competing priorities across multiple regions.

About you:

  • Auslan fluency
  • Have at least 2+ years’ experience working within the Support at Home and/or Commonwealth Support programs
  • Have strong knowledge of aged care compliance and regulatory requirements
  • Are a real team player who collaborates positively with colleagues and stakeholders
  • Can multitask effectively and thrive in busy environments
  • Work well under pressure and adapt to changing priorities
  • Are organised and confident managing documentation and case notes
  • Communicate effectively with a range of stakeholders
  • Are committed to person-centred, goal-focused care
  • Are comfortable supporting clients across NSW, VIC, SA and WA
